Consent: I am the parent or legal guardian of the child(ren) listed below, and I am informed of the activities offered by Bethel Lutheran Church at 1321 North Avenue, Northfield, MN, beginning August 1, 2023 and ending July 31, 2024.

Photo opt OUT? Occassionally we post pictures on our website and Facebook page of activities and events that take place at Bethel. If we do not have your permission to post a picture with your child, please indicate so below. We will never attach names to any photo to identify participants.

Please add a photo of your child. This can be a snap shot or a school photo but please choose a photo where you can clearly see their face. This will be used for a couple of things. First, so teachers (and Kristi) can quickly learn the names of their students. 8th grade student's photos will be given to their prayer partner as well and 9th grade students to their mentors.
